Our practice is proud to offer you a new care management program, called ChartSpan.

Created for individuals with Medicare, care management gives you access to a nurse line 24

hours a day, 7 days a week, along with monthly support in reaching your care goals. You can

call with any medical questions or concerns you have.

Your personal care coordinator will help you set up appointments, refill your prescriptions,

create a care plan, find community resources, and work toward specific health goals.

Medicare created care management and pays for these services, though you may have a

monthly copay. ChartSpan will call soon to give you more information and ask if you want to

enroll. You may only be enrolled under one provider and can unenroll at any time. If you’d like
